DOCUMENTS REQUIRED FOR TRANSPORT DAMAGES

When it is domestic shipping;

  • Statement of the insured / policy holder regarding the damaged commodity
  • Original or photocopy of the policy
  • Original or copy of commodity invoice
  • Original or copy of packing slip
  • Copy of Freight (Transport) invoice
  • Traffic accident report / alcohol examination (if there is a traffic accident)
  • Records prepared by Official Authorities regarding the damage, if there is any
  • Damage assessment report (containing the truck / lorry plate and the driver’s signature)
  • Damage Notification Letter sent to the carrier company from the notary public by the insurance / policy holder (Warning Statement)
  • List of Authorized Signatures
  • Pictures of damaged goods
  • Bank details of the insured
  • IBAN information
  • Other (additional) documents deemed necessary by the insurer according to the nature of the damage

When it is international shipping;

  • Statement of the insured / policy holder regarding the damaged commodity
  • Original or photocopy of the policy
  • Original or copy of commodity invoice
  • Bill of lading in sea transport, bill of lading or CMR in international road transport, airway bill in airline, railway bill in railway
  • Customs entry declaration (in case of import)
  • Customs exit declaration (in case of export)
  • Damage assessment report (containing the signature of the carrier)
  • Records prepared by Official Authorities regarding the damage, if there is any
  • Towing list
  • Transport contract, freight (transportation fee) invoice
  • Damage notification letter sent to the carrier company from the notary public by the insurance / policy holder (Warning Statement)
  • IBAN information
  • Pictures of damaged goods
  • Bank details of the insured
  • Other (additional) documents deemed necessary by the insurer according to the nature of the damage

When it is Domestic Carrier Liability Insurance Damages;

  • Statement of the insured / policy holder regarding the damaged commodity
  • Original or copy of the policy
  • Original or copy of commodity invoice
  • Original or copy of packing slip
  • Copy of the freight (transport) invoice
  • Traffic accident report / alcohol examination (if there is a traffic accident)
  • Records prepared by Official Authorities regarding the damage, if there is any
  • Damage assessment report (containing the truck / lorry plate and the driver’s signature)
  • Pictures of damaged goods
  • Documentation of the transport vehicle
  • Invoice and letter certifying that the owner keeps the carrier responsible for the damage within the framework of the legal liability of the carrier
  • IBAN information
  • A copy of the shipping commodity insurance for the commodity, if there is any, or the owner’s statement that there is no commodity policy
  • Bank details of the insured
