DOCUMENTS REQUIRED FOR COMPREHENSIVE INSURANCE CLAIMS
- Traffic Accident Report / Agreed Accident Report / Statement and Witness Report
- Declaration in cases where there is no report or official record

IN CASE OF VEHICLE THEFT:
- Police report of the theft
- Documents from the police station and auto theft bureau stating that the vehicle has not been found (if not found within 30 days from the date of theft)
- Original vehicle registration with “stolen” stamp
- Consent from the insured for payment if there is a lien on the vehicle, along with a notarized document confirming the removal of the lien
- Tax clearance certificate
- Letter from the traffic registration office confirming the removal of the vehicle from the traffic registry
- Power of attorney (to be obtained from our company)

In the event of the theft of a radio or tape player, the insurance coverage provided for the radio or tape player in your policy will be terminated. Therefore, if you install a new radio or tape player, additional coverage must be added to your policy.

DIMINISHED VALUE CLAIMS

FOR PROPERTY DAMAGE:

- Photos of any damage caused by the theft

ENGINEERING
FOR ELECTRONIC EQUIPMENT INSURANCE CLAIMS:
- Insured’s statement and relevant reports related to the incident
- Technical service report (indicating the cause of the damage)
- Fire department report if the damage was caused by a fire
- Police report and “Not Found” letter if the damage was caused by theft
- Purchase invoice of the damaged equipment
- Photographs
- Repair invoice if repairs have been made
- Maintenance contract and periodic maintenance records

CONSTRUCTION INSURANCE CLAIMS

MACHINE BREAKDOWN INSURANCE CLAIMS

CARGO CLAIMS
For Domestic Shipments:
- Insured’s/Policyholder’s written statement regarding the damaged goods
- Original or copy of the policy
- Original or copy of the goods invoice
- Original or copy of the shipment waybill
- Copy of the freight (transportation) invoice
- Traffic accident report / alcohol test (if there was a traffic accident)
- Any official reports issued by authorities regarding the damage
- Damage assessment report (including the truck/trailer license plate and the driver’s signature)
- Damage Notification letter sent by the insured/policyholder to the carrier via notary (Notice)

For International Shipments:

- Customs import declaration (in case of import)

For Domestic Carrier Liability Insurance Claims:

- Copy of the freight (transportation) invoice
- Traffic accident report / alcohol test (if there was a traffic accident)

- Documents related to the transport vehicle
- Invoice and letter from the goods owner holding the carrier responsible for the damage under legal liability

- In case the parties cannot reach an agreement and the matter is taken to court, the court ruling
- If a payment has been made by our insured based on the court ruling, the execution order and payment receipt

FIDELITY (EMPLOYEE DISHONESTY) CLAIMS
- Insured’s statement detailing the amount claimed, the nature of the incident, and bank details for compensation payment
- Employment notification form (Social Security registration) of the employee who committed the act of dishonesty
- Employment contract
- Payroll
- Criminal complaint filed with the Public Prosecutor’s Office
- Document detailing the outcome of the investigation conducted by the Prosecutor’s Office
- Written and signed statement of the employee taken before the Prosecutor or otherwise
- Witness statements
- Documents related to any bonuses, incentives, or other entitlements earned by the employee from the start of employment until the date of the incident
- Accounting records and documents showing the amount embezzled
- Document/notice of the employee’s dismissal
- Documents and information related to any deductions made from the employee’s pay or amounts recovered in cash or otherwise in relation to the loss

LOSS OF PROFIT CLAIMS
General Documents:
- Corporate Tax Returns and their annexes for the three years preceding the date of loss
- Provisional Tax Returns and their annexes for the three years preceding the date of loss
- Budget for the year prior to the date of loss and the budget for the year in which the loss occurred
- Certified Public Accountant’s Certification Report of Corporate Tax Returns for the three years preceding the date of loss
- Capacity Reports for the three years preceding the date of loss
- Official inventory book for the three years preceding the date of loss
- Investment Incentive Certificates
- Independent Audit Reports for the three years preceding the date of loss
Accounting Documents:
- Balance Sheets and Income Statements (submitted to the Tax Office) for the three years preceding the date of loss
- Provisional Tax Returns and their annexes, Income Statements, and detailed general ledgers showing costs for the three years preceding the date of loss
- Monthly detailed general ledgers (non-cumulative) for the three years preceding the date of loss
- Monthly detailed general ledgers (non-cumulative) for the one-year period prior to the date of loss
- Value Added Tax Returns for the three years preceding the date of loss
Planning and Costing Documents (Management Accounting Documents):
- Information on the production cost system used for the damaged machines as of the date of loss and the documents related to this cost system
- Detailed stock lists related to production; if unavailable, non-accounting lists of volume-profit analyses of the machines as of the date of loss
- Detailed lists of the unit and amount of stocks at the beginning of the period, as well as at the start and end of the loss date (also in detailed general ledgers)
- Detailed physical stock count lists for the three years preceding the date of loss
